• CTA Community Supports (268-4887 in Salt Lake City) provides residential support, family support, day support, extended residential support, and supported employment for adults age 22 years and over.
• Chrysalis provides supported employment.
• DDI Vantage (269-8108 in Salt Lake City) provides supported employment and supported job based training for people with disabilies.

• Future Through Choices (292-0068 in Bountiful) provides residential support services and pre-vocational skills for individuals with developmental disabilities.

We believe people are best supported in families, homes of their own, vocational and small residential settings in the community. Our services and training are as varied as the people we support.
• RHA/Community Services of Utah (582-8097 in Salt Lake City) for people with developmental disablilites focusing on medical, recreational, pre-vocational, behavioral, daily living skills, and basic survival skills.
• Self-Determination, Inc. (274-6058 in Holladay) is a nonprofit organization designed to assist adults with mild to moderate cognitive disabilites with independent living skills, behavior, and employment.
• (359-8876 in Salt Lake City) is a nonprofit organization which offers community living supports, supported living and personal assistance, host home and professional parent services, companion services, day supports, supported employment, respite care, summer programs, transportation, etc.
• Valley Employment Services (293-9521 in Salt Lake City) provides time limited training and support in job placement, on-the-job training, job coaching, etc.
• Work Activity Center (977-9779 in West Valley City) provides supported job based training, supported employment, and community based training for individuals with disablilties.

Some individuals benefit greatly from adult service providers. Service could include: supported employment, residential care, day care, advocacy, family support, and consultation.
