STUDENT DISCIPLINE POLICY

As a Community of Caring School, we teach the values of Respect, Caring, Responsibility, Trust, and Family. We will not tolerate incidences of disrespectful language, either joking or serious. Disrespectful words or actions are unacceptable, especially those focused on race, ethnicity, sexual orientation, gender, religion, social class, appearance, and disability.

Our STOP AND GO discipline policy helps students ensure that you child's learning opportunities at Fremont Elementary are safe and positive.

 
GUIDELINES FOR BEHAVIOR
1. I will be kind to myself, others, and my environment.
2. I will respect and follow adult guidance at all times.
3. I will allow others to work and learn peacefully.
 
STOP AND GO POLICY
 
STOP AND THINK: If a student does not follow school, classroom, or playground rule, he/she will be asked by school staff to go to Stop & Think. Students should go quietly to the designated area in the classroom or playground within 10 seconds when directed by a supervising adult. The student will remain in the Stop & Think area for five minutes or until their behavior is under control. Students may be given two Stop & Think opportunities to correct their behavior within a set period of time designated by the grade level. The supervising adult will suggest ways for the student to modify his/her behavior and make certain that the student understands what rule is not being followed.
 

STOP AND GO: If a student is unable to modify his/her behavior following two Stop & Think opportunities, the child will be directed to Stop & Go. Students who are defiant, aggressive, disrespectful, or verbally abusive will not be given Stop & Think opportunities, but will immediately be sent out of their own classroom to a Stop & Go area. When directed, the students will quietly take a Stop & Go form and their class work to the designated Stop & Go classroom within 30 seconds. If the child refuses to follow the teacher's instruction to move to a Stop & Go area away from their peers, a second Stop & Go will immediately follow and the office may be called for intervention. The student will remain under the supervision of the designated supervisor for a developmentally appropriate period of time. Older children will remain for at least 30 minutes and until their work is completed. The homeroom teacher will inform the child's parents by sending the Stop & Go form home with the child at the end of the day. It is the student's responsibility to give the note to their parent/guardian and have it signed. The parent signature merely denotes the note was delivered. Students who fail to return the signed note will lose priveleges.

 
REFERRALS TO PRINCIPAL: Any student who obtains three Stop & Go directives within a one month period of time will be referred to the principal. The principal will notify the child's parents and take disciplinary steps up to and including suspension. Multiple infractions or multiple referrals will lead to more serious consequences. Any behaviors which may endanger students or adults will be immediately referred to the office for intervention and will be dealt with under the Granite District Safe Schools Policy.

SCHOOL LUNCH

School lunch and breakfast can be purchased on a daily, weekly, monthly, or yearly basis.    We encourage you to pay by the month or week whenever possible.  Each lunch costs $1.20 and breakfast cost $0.75; milk purchased separately is 30˘.  Application forms for free and reduced lunch and breakfast are available in the office.  Lunch and breakfast money can be paid in the student’s classroom or in the office.  Milk money is paid in the lunchroom.

Lunches and breakfast are served on a pay-as-you-go basis.  Elementary schools allow up to two temporary meal charges for students who do not have lunch money, but we cannot allow students to continue to eat if the money is not reimbursed.  Secretaries hand students a reminder note as they go through the lunch line if advance-purchased lunches are running out.

 

ACCIDENTS

Injuries of a minor nature, such as a skinned  knee, will be cared for by school staff.  (The school does not have a nurse.)  If a child has a more serious injury, the parent or guardian will be called first and if he/she is unavailable, the designated emergency contact will be phoned.  If no one is available and the student appears to have a serious problem, emergency units may be called.

 

SEP CONFERENCES

Student Education Plan (SEP) Conferences are meetings to review progress and set individual goals.  Three set of conferences will be held during this school year.  Students are expected to attend with their parents or guardians.

HOMEWORK POLICY

Homework assignments should be enjoyable experiences for children, giving them opportunities to demonstrate creativity through individual effort and by sharing experiences with family members.  Homework should be in accordance with the general needs of the child.  Homework at Fremont may encompass: providing additional time for students to complete work or special assignments that can’t be finished in class; reinforcing skills taught during the day; completing make-up work when students have been absent; providing enrichment experiences.

HEALTH INFORMATION

If a student has a special health problem, the parent/guardian should notify the teacher and the office.  Written instructions, including a telephone number and person to contact in an emergency, should be provided for children who are diabetic, convulsive, have severe allergy disorders, or other serious problems.

Prescription medication should be administered at home whenever possible.  Parents of students who need to have medication administered at school must obtain a copy of the medication policy and “Permission to administer Medication” from the office.  This form must be completed by the parent and physician before medication can be given at school.  Students should not carry prescription medication to and from school.  The medication must be delivered to the school in a labeled pharmaceutical container by the parent.  No medications, including aspirin or Tylenol, can be administered at school without a copy of the “Permission to Administer Medication” form filed in the office.

 

ACCIDENT INSURANCE

Accident insurance for students is available for purchase each year through a private vendor.  If you do not have insurance, this is an excellent opportunity for coverage.  Application forms are given out during registration in the fall.  After that, application forms can be obtained at the office.

 

IMMUNIZATION RECORDS

Utah state law requires each child to have an up-to-date immunization record on file at the school before the beginning of school.  If a parent chooses not to have their child immunized, they must go to a Salt Lake County Family Health Clinic to complete the official Utah Department of Health Personal Exemption Form.  The following immunizations are required:

4 DPT (A 5th dose is required if 4th dose was prior to the 4th birthday) or DT, 3 doses
3 Polio
2 MMR (3rd dose is required if the 1st dose was prior to the 1st birthday)
3 Hepatatis B --
2 Hepatatis A -- (Kindergarten , First, Second, & Third Grades only)
1 Varicella (Chickenpox) -- (Kindergarten First, Second, & Third Grades only)

Any immunization record provided by a licensed physician, registered nurse, or public health official is acceptable.  Parental records can be used if they include the day, month and year for administration for each type of immunization and the name of the physician or agency administering the vaccine. 

If a student’s immunization record in not complete, the parents will be notified.  If students are not completely immunized against each specified disease, they can be enrolled while in the process of receiving immunizations.  Students without appropriate documentation of adequate immunization will be excluded from school.  Parents will be notified in writing before a child is excluded.

 

DRESS CODE

Appropriate school dress is the responsibility of parents and students. Students should dress comfortably and wear clothing that will allow them to go outside in the prevailing weather conditions.  Clothing should be safe, modest, clean.  Students may not wear tank tops, midriff tops, bandanas, or hats.  Shorts should be fingertip length and midriffs should be covered.  Any clothing item which imitates “gang” fashion, advertises alcohol or tobacco, or has suggestive sayings or illustrations is not appropriate. If necessary, parents will be contacted to provide appropriate clothing.

ATTENDANCE

School attendance is required by law.  Students should be in school except in cases of illness and emergencies, medical appointments, death of a family member or close friend.  Appointments, whenever possible, should be scheduled outside of school hours.

When your child is absent, a note explaining the absence is required. If a note is not sent to school, we assume the absence to be unexcused.

Children should arrive at school five to ten minutes before the tardy bell rings. Students arriving after 10:00 a.m. must report to the office to obtain a check-in note.

Students leaving school during the day must be checked out in the office.  Telephone calls requiring the release of a student will not be honored unless the office staff can positively identify the caller.  Students are not permitted to leave the grounds during school hours without being checked out by a guardian.

No supervision is provided on the playground before or after school. Children are allowed to enter the building and go to the multi-purpose room at 8:50 if the weather is inclement.

LOST AND FOUND

We make every effort to promote honesty and respect for others’ property.  However, the school cannot assume responsibility for losses.  Please place identification labels on personal property your child brings to school and use caution in allowing your child to bring valuable items.  Parents are always welcome to check the lost and found bin for lost items.

 

VISITORS TO THE SCHOOL

Parents are always welcome to the visit the school and we are pleased to have volunteer help.  We do require that any visitor come to the office and pick up a visitors badge before entering any classroom.  Lengthy visits require administrative approval.

 

SCHOOL TELEPHONE

The school phone is a business phone and is not to be used for students to arrange to go home with friends.  Student use of the phone is restricted to emergencies only.  We encourage parents not to call students at school unless it is an emergency.  Giving messages to children during the day disrupts instructional time.

 

GRANITE DISTRIC SCHOOL SAFETY POLICY

For the safety of students, staff, and visitors to our schools, the Granite School District Board of Education has adopted a strict SCHOOL SAFETY POLICY which absolutely prohibits the following:

Any threat or act of violence
Possession of any type of weapon, real or facsimile, regardless of intent
Any form of gang related activity
Any type of criminal behavior
Any destruction or defacement of school property
Willful disobedience or persistent defiance of authority.
 
Violators of this policy may subject themselves to the following:
Immediate request for police investigation
Automatic suspension from school
Mandatory parent or guardian conferences with school or district authorities
Referral to district Pupil Services Office for placement determination.
 
In cases involving severe or repeated violations, the student:
May be placed in an alternative program
May be referred to other programs in an effort to resolve the problem
May be expelled from school. (A one year expulsion is mandatory for any incident involving firearms.)
