STUDENT DISCIPLINE POLICY

Fremont Elementary provides children with a positive, safe learning environmnet. Our STOP AND GO discipline policy helps students develop a sense of responsibility for their own behavior. The program is in force durring the school day, going to and from school, and durring all school sponsored activities. As a Community of Caring School, we teach the values of Respect, Caring, Responsibility, Trust and Family. We will not tolerate incidences of disrespectful language, either joking or serious. Disrespectful words or actions - especially those focused on race, ethnicity, sexual orientation, gender, religion, social class, appearance and disability - are unacceptable.

STOP AND GO POLICY
STOP AND THINK: A student who is not following the rules will be directed to go to a stop and think area of the classroom or playground. He/she must go silently within 5 seconds of the direction. The student will remain in the area for five minutes or until their behavior is under control.
 

STOP AND GO: A student who is defiant, aggressive, or verbally abusive, or a child who does not modify his/her actions after two stop and think directions will be given a stop and go direction. The student is to take his/her work and go to a designated area in another classroom or be sent in from the playground. The student will remain with the stop and go teacher for at least 15 minutes, depending on their age. Homeroom teachers will send the stop and go form home with the student for the parent to sign.

 
OFFICE INTERVENTION: A student who obtains three Stop & Go directions within a one month period of time will be referred to the principal. Parents will be notified and a conference may be scheduled with the principal. Multiple referrals will lead to more serious consequences. Disciplinary action, up to and including suspension, will be considered.
 
SERIOUS OFFENSES: Any behaviors which may endanger students or adults will immediately be referred to office and may be dealt with under the Granite District Safe School Policy.
 

SCHOOL LUNCH

School lunch and breakfast can be purchased on a daily, weekly, monthly, or yearly basis.    We encourage you to pay by the month or week whenever possible.  School lunch (including milk) costs $1.40 and breakfast cost $0.85; milk purchased separately is 30˘.  Application forms for free and reduced lunch and breakfast are available in the office.  Lunch and breakfast money can be paid in the office.  Milk money is paid in the lunchroom. If your child needs a special diet please see the lunch secretary for a form.

Lunches and breakfast are served on a pay-as-you-go basis.  Elementary schools allow up to two temporary meal charges for students who do not have lunch money, but we cannot allow students to continue to eat if the money is not reimbursed.  Reminders are given to students daily if their money for advanced purchased lunches are low.

ACCIDENTS

Injuries of a minor nature, such as a skinned  knee, will be cared for by school staff.  (The school does not have a nurse.)  If a child has a more serious injury, the parent or guardian will be called first and if he/she is unavailable, the designated emergency contact will be phoned.  If no one is available and the student appears to have a serious problem, emergency units may be called. The school needs current phone numbers !!!

SEP CONFERENCES

Student Education Plan (SEP) Conferences are meetings to review progress and set individual goals.  Three set of conferences will be held during this school year.  Students are expected to attend with their parents or guardians.

HOMEWORK POLICY

Homework assignments should be enjoyable experiences for children, giving them opportunities to demonstrate creativity through individual effort and by sharing experiences with family members.  Homework should be in accordance with the general needs of the child.  Homework at Fremont may encompass: providing additional time for students to complete work or special assignments that can’t be finished in class; reinforcing skills taught during the day; completing make-up work when students have been absent; providing enrichment experiences.

HEALTH INFORMATION

If a student has a special health problem, the parent/guardian should notify the teacher and the office.  Written instructions, including a telephone number and person to contact in an emergency, should be provided for children who are diabetic, convulsive, have severe allergy disorders, or other serious problems.

Prescription medication should be administered at home whenever possible.  Parents of students who need to have medication administered at school must obtain a copy of the medication policy and “Permission to administer Medication” from the office.  This form must be completed by the parent and physician before medication can be given at school.  Students should not carry prescription medication to and from school.  The medication must be delivered to the school in a labeled pharmaceutical container by the parent.  No medications, including aspirin or Tylenol, or other OTC medications can be administered at school without a copy of the “Permission to Administer Medication” form filed in the office.

ACCIDENT INSURANCE

Accident insurance for students is available for purchase each year through a private vendor.  If you do not have insurance, this is an excellent opportunity for coverage.  Application forms are given out during registration in the fall.  After that, application forms can be obtained at the office.

IMMUNIZATION RECORDS

Utah state law requires each child to have an up-to-date immunization record on file at the school before the beginning of school.  If a parent chooses not to have their child immunized, they must go to a Salt Lake County Family Health Clinic to complete the official Utah Department of Health Personal Exemption Form.  The following immunizations are required:

4 DPT (A 5th dose is required if 4th dose was prior to the 4th birthday) or DT, 3 doses
3 Polio
2 MMR (3rd dose is required if the 1st dose was prior to the 1st birthday)
3 Hepatatis B
2 Hepatatis A
1 Varicella (Chickenpox)

Any immunization record provided by a licensed physician, registered nurse, or public health official is acceptable.  Parental records can be used if they include the day, month and year for administration for each type of immunization and the name of the physician or agency administering the vaccine. 

If a student’s immunization record is not complete, the parents will be notified.  If students are not completely immunized against each specified disease, they can be enrolled while in the process of receiving immunizations.  Students without appropriate documentation of adequate immunization will be excluded from school.  Parents will be notified in writing before a child is excluded.

DRESS CODE

Appropriate school dress is the responsibility of parents and students. Students should dress comfortably and wear clothing that will allow them to go outside in the prevailing weather conditions.  Clothing should be safe, modest, clean.  Students may not wear tank tops, midriff tops, bandanas, or hats.  Backs and midriffs must be covered at all times. Shorts should be fingertip length plus one inch.  Pants should not hand so low that underwear is showing. Any clothing item which imitates “gang” fashion, advertises alcohol or tobacco, or has suggestive sayings or illustrations is not appropriate. Any fashion which distracts from the learning environment is inappropriate. Hair should be natural colors. If necessary, students will be removed from class and parents will be contacted to provide appropriate clothing.

CELL PHONE & ELECTRONIC DEVICES

The Granite School Board has approved a policy affecting students use of cell phones, pagers, and other electronic communication devices. These services are not to be used or in the student's possession durring school hours. If a student is found to have one of these devices, it will be confiscated and only be returned after a parent meeting. The school and district accept no responsibility for the loss or damage to any communication devices.

ATTENDANCE

If your child is going to be absent, you are required to call the attendance office at (385) 646-4838, this phone number is available 24 hours per day, 7 days per week.

School attendance is required by law.  Students should be in school except in cases of illness and emergencies, medical appointments, death of a family member or close friend.  Appointments, whenever possible, should be scheduled outside of school hours. If this is not possible and your child has missed any part of a school day, please bring a physicians note stating the day your child was seen and when they can return to school.

When your child is absent, and not seen by a physician, a note explaining the absence is required. If a note is not sent to school, the absence is considered to be unexcused.

A parent can excuse their child up to 7 (parent excused) days per school year.

Children should arrive at school five to ten minutes before the tardy bell rings. (Tardy Bell rings at 9:05) No supervision is provided on the school ground before or after school. Children not eating breakfast are allowed to enter the building at 8:50 if the weather is inclement. Students who arrive after 9:05 am must report to the office for a late slip.

Students leaving school during the day must be checked out in the office by a guardian, they may not leave school grounds otherwise.  Telephone calls requiring the release of a student will not be honored unless the office staff can positively identify the caller.

LOST AND FOUND

We make every effort to promote honesty and respect for others’ property.  However, the school cannot assume responsibility for losses.  Please place identification labels on personal property your child brings to school and use caution in allowing your child to bring valuable items.  Parents are always welcome to check the lost and found bin for lost items.

VISITORS TO THE SCHOOL

Our school has a new security system. Starting this year, the only doors open to visitors will be the main front doors. All other doors will have to be opened with a district pass. This is to ensure the safety of our students, as well as to direct any visitors to the respective areas. Please understand that this new system will benefit our school and your students, limiting access from strangers and helping the teachers have better knowledge of students leaving their class. Parents are always welcome to the visit the school and we are pleased to have volunteer help.  We do require that any visitor come to the office and pick up a visitors badge before entering any classroom.  Lengthy visits require administrative approval. If there are any questions, please feel free to call the school.

SCHOOL TELEPHONE

The school phone is a business phone and is not to be used for students to arrange to go home with friends.  Student use of the phone is restricted to emergencies only.  We encourage parents not to call students at school unless it is an emergency.  Giving messages to children during the day disrupts instructional time.

EMERGENY INFORMATION

Fremonts emergency plan is available in the office. In an emergency where the school must be evacuated, students will walk to Eisenhower Jr High School. Parents and Guardians may come to the auditorium to sign students out of school. Please be sure the school has each students current phone number and address in case of emergency.
