Registration:

In order to register your child to attend D. T. Orchard Elementary, you will need to take care of several things.

1) You'll need to check out of the current school is your child is currently enrolled. They will give you a Transfer form that you'll bring with you to register.

2) Gather a copy of your child's Birth Certificate, Immunnization, and proof that you live within the school boundaries. A Utah Birth Certificate can be obtained through the Utah Department of Health. The Immunization form for students new to Kindergarten is here. The parent's immunization card/record is fine for students already attending school. As proof of residency, you many bring a current utility bill, housing contract or other real estate forms that show your current address.

3) You'll need to fill out the Registration Form. This can be done at the school, or can be downloaded and completed ahead of time.

4) Bring the Birth Certificate, Immunization, proof of residency and Registration Form (may be done at school) to the Office at the school. We will help you complete the registration.

Because classes are already established, you will probably not have a choice of tracks. We will keep your family together on the same track, unless you want to have your children on separate tracks.

Your child will begin on the next school day for the assigned track after registration is completed.
