Disclosure Statement
2004-2005 School Year
Ms. Vaughn
As a teacher of Pre-Algebra, Reading & Language Arts classes, I care about
your student and I want them to learn. I feel that this is each student’s
right. I will do everything I can to provide a safe environment where I can
teach and students can learn. In order to provide the most stimulating learning
environment for your child, the following procedures and policies will be used
and implemented in my classroom.
TO SECURE SAFE LEARNING ENVIRONMENTS STUDENTS ARE REQUIRED TO
CRITICAL LEARNING HABITS/CLASSROOM RULES
• Follow directions the first time given.
• Be in your seat, prepared with all required supplies when the bell rings, or
you are tardy!
• Eliminate ridicule, sarcasm, snickering, and put-downs.
• Ask permission, and honor the reply, before touching others or their
possessions.
• Practice “Teacher Time.”
CONSEQUENCES FOR CHOICES THAT PROMOTE LEARNING
• Praise and recognition
• Positive notes and phone calls home
• Non-disruptive free time

Resource Department
Where possible, the material and the way it is presented to the student
will be individualized to reflect the goals stated in the student’s Individual
Education Plan (IEP). A report will be sent home if it becomes evident that a
student is in danger of failing. A copy of that report will be on file in the counseling
office.
Students will have daily classroom assignments. If they do not finish them in
class, they will be expected to finish them at home (homework). Note: As a
general rule I do not routinely give homework assignments. Students have one
week for each day missed to make up work. Missed work must be turned in before
missed testes are given. Missed tests will be scheduled with the teacher for
make up before or after school. It is the student’s responsibility to check
with the teacher after an absence for all missed assignment / tests. All
assignments are due on the due-date given in class.
Behavior, participation, preparedness, tardies, and
attendance determine citizenship. The school-wide rules and tardy policy will
be followed and enforced. Excessive tardies and/or
absences can result in students missing vital class-student interaction, class
participation, and essential information.
Please feel free to call (801) 250-8645 if you have questions or concerns. If
the teacher is not available, you may leave a message at the front office to be
returned as soon as possible.
