The Utah State Board of Education and the State Department of Health are administering a statewide survey to students in grades 6, 8, 10, and 12.
The Student Health and Risk Prevention Survey (SHARP) survey is administered every other year, and is designed to assess adolescent substance use, anti-social behavior as well as risk and protective factors that can predict problem behaviors in youths.
We strongly encourage you to review the materials carefully to ensure you are informed regarding its content prior to giving your consent. Additionally, since the survey covers sensitive topics regarding risky adolescent behaviors, we would also encourage you to follow up with your own child to discuss their specific needs and challenges.
