Biometric Screening<\/h2>\n\n\n\n

All contract employees on one of the district’s medical insurance plans are required to have a biometric screening done every plan year. The biometric screening can be performed by your personal physician or completed at the Wellness Center. If completed by your personal physician a physician form must be completed and returned to the benefit office. If completed at the Wellness Center, a file will be sent to the benefit office no physician form required. Employees will have January 1 through December 31 to complete their biometric screenings for the following insurance plan year. Employees who do not complete the biometric screening will be charged an extra $10 a month for medical premiums for the plan year.<\/p>\n\n\n\n
