**Your application form has been submitted; HOWEVER, this is just the first step in the process!
YOUR CHILD WILL NOT BE PLACED IN A CLASS until we receive the following information:
If you have registered for a Limited Admission school you must provide:
- a copy of the birth certificate (not required for returning students)
- a copy of the UTAH immunization card (not required for returning students)
- a copy of CURRENT proof of address (This proof must show the address as “service address” or “property address.” It can be a mortgage statement or rental agreement, or a water, gas or electricity bill in one of the parents’ names.)
If you have registered for an Open Admission school you must provide:
- a copy of the birth certificate (not required for returning students)
- a copy of the immunization record (not required for returning students)
- a $40 non-refundable registration fee and the first month’s tuition. (Pay by calling our office with a credit or debit card.)

Thank you for registering your child! We will mail you a confirmation of class placement once your registration is complete. If your requested class is full, you will be placed in a comparable class.
Confirmations can typically take up to 2 weeks AFTER we receive the items listed above. If you have not heard from us after 2 weeks, please contact our office at 385-646-4672.
