{"id":606,"date":"2018-09-10T13:11:03","date_gmt":"2018-09-10T19:11:03","guid":{"rendered":"https:\/\/www.graniteschools.org\/psp\/?page_id=606"},"modified":"2026-03-09T12:10:10","modified_gmt":"2026-03-09T18:10:10","slug":"homebound-hospital","status":"publish","type":"page","link":"https:\/\/www.graniteschools.org\/ssp\/homebound-hospital\/","title":{"rendered":"Homebound\/Hospital"},"content":{"rendered":"<p>The Granite School District Homebound\/Hospital Program provides temporary home or in-hospital instruction to Granite School District students who are unable to attend school for an extended period of time (minimum three weeks) due to illness or injury.<\/p>\n<p><strong>Instructions to Parents\/Guardians<\/strong><\/p>\n<p>If your child is ill or injured and you believe he\/she is not able to attend school for an extended period of time (3+ weeks), please take the following steps:<\/p>\n<ol>\n<li>Meet with a school representative (counselor, social worker, psychologist, or administrator) at your child\u2019s school to inform them about your child\u2019s situation.<\/li>\n<li>The school representative will review the information you have provided with the school\u2019s Student Support Team (SST) that includes administrators, counselors, social worker, psychologist, school nurse, and\/or teacher representatives.<\/li>\n<li>The SST will discuss possible supports, interventions, and\/or accommodations to help your child continue his\/her education during the period of illness or injury.<\/li>\n<li>A school representative will contact you with the Team\u2019s plan of action to help your child.<\/li>\n<li>If the plan of action is to refer to Homebound\/Hospital, the school will provide you with the needed paperwork.<\/li>\n<li>Once the paperwork is completed and returned to the school, they will forward it to the Homebound\/Hospital Office and a teacher will be assigned to contact you and arrange for a time and place for instruction.<\/li>\n<li>Homebound\/Hospital teachers are allowed up to a maximum of two hours per week to instruct your child. They will assign work for your child to do during the week between visits.<\/li>\n<li>For students requiring a long-term Homebound\/Hospital placement (more than nine weeks), a renewed <strong>Physician\u2019s Recommendation Form<\/strong> will be required at least every nine weeks.<\/li>\n<\/ol>\n<p><strong>Instructions to School Personnel<\/strong><\/p>\n<p>Please note: A note from a health-care professional recommending home\/hospital is not a guarantee of that service. Your SST may decide that other interventions\/accommodations may be more appropriate for the student. In that case, it is the responsibility of the SST to reach consensus with the health-care professional and the parent.<\/p>\n<p>All Home\/Hospital referrals must originate from the student\u2019s school. When you become aware that a student might be in need of a Home\/Hospital placement, please take the following steps:<\/p>\n<ol start=\"1\">\n<li>Meet with the parent\/guardian to review and sign the\u00a0<strong>Student Support Form <\/strong>(pink sheet),<\/li>\n<li>Review the \u201cpink sheet\u201d with your SST to get a team decision on what is the appropriate intervention\/accommodation for the student.<\/li>\n<li>If the SST decides that Home\/Hospital is the appropriate placement for the student, complete the\u00a0<strong>SST Home Hospital Recommendation Form<\/strong> and have it signed by SST stakeholders.<\/li>\n<li>Assign an SST member to contact the parent\/guardian to inform them of the SST decision.<\/li>\n<li>Give the parent a Home\/Hospital\u00a0<strong>Physician&#8217;s Recommendation Form<\/strong> to be completed by the health-care professional and returned to you.<\/li>\n<li>Have the parent\/guardian sign the <strong>Parent Release of Information Form.<br \/>\n<\/strong><\/li>\n<li>Send these completed forms to the Home\/Hospital Office*:\n<ul>\n<li>A copy of the <strong>SST Home\/Hospital Recommendation Form<\/strong>.<\/li>\n<li>A copy of the Home\/Hospital <strong>Physician&#8217;s Recommendation Form<\/strong>.<\/li>\n<li>A copy of the <strong>Parent Release of Information<\/strong>.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><a href=\"https:\/\/www.graniteschools.org\/psp\/wp-content\/uploads\/sites\/33\/2022\/09\/Combined-Home-Hospital-Forms.pdf\"><strong>Access the forms here (PDF)<\/strong><\/a><\/p>\n<p><strong>Please Note<\/strong>:\u00a0 All Home\/Hospital referrals for students with I.E.P.s must be reviewed and approved by a district special education coordinator before submitting the referral to Home\/Hospital.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>The Granite School District Homebound\/Hospital Program provides temporary home or in-hospital instruction to Granite School District students who are unable to attend school for an extended period of time (minimum three weeks) due to illness or injury.
