Fundraiser & Commercial Co-Venturer Application

As part of our district fundraising policy and procedures and to comply with Utah Code Annotated §13-22 Charitable Solicitation Act and Utah Administrative Code R152-22, all vendors are now required to utilize the following application process prior to the fundraiser being utilized within our schools. Please complete the following application process. You will be notified upon successful admission and approval. If you have any questions, please email Jared Gardner.

** Please note that fundraising activities where a student group performs a service in return for a wage is classified differently than typical fundraising. Vendors in this category do not need to submit an application, but will work directly with the school. **

Vendor Fundraiser & Commercial Co-Venturer Application

  • This is the employer's Federal Tax ID Number.
  • Required Documentation

    • - If you have a contract or ordering document that will need to be signed by the district/school/team/club (District), it must be submitted with your application.
    • - Any applicable terms and conditions that will apply to the contract or ordering document must also be submitted.
    • - Terms and conditions on your company’s webpage that would bind the District must be submitted.
    • - State of Utah fundraiser permit, commercial co-venturer letter from the Utah Division of Commerce, or a letter of exemption from the Utah Division of Commerce must be submitted.
    • - A copy of the organization’s current valid business license.
    Note: For uploads, a .pdf file is best, but .png and .jpg files are also accepted.

  • Your application will be rejected if the permit, commercial co-venturer letter, or letter of exemption is not attached or if the submitted permit has expired.

  • Agreements

    These agreements are part of the Granite School District's fundraising policy. Violation of these assurances will remove you as a district-approved fundraising partner.
