Thank you in advance for participating in the College and Career Ready Program Questionnaire. Granite School District’s comprehensive counseling program is the structure through which school counselors deliver College and Career Readiness Counseling to all students in their school.
The purpose of this questionnaire is to obtain information about how the school counseling program can be more effective in helping your student become college and career ready.
The questionnaire takes approximately 5 to 10 minutes to complete and is no more than 40 questions. Your name and contact information will NOT be recorded with the answers to your questions. All submissions of this questionnaire will be anonymous.
The questionnaire is most easily completed online at the following web address.

You may simply complete the questionnaire online, print off the confirmation page and return it to your student’s school.
If you do not have access to a computer or would prefer to complete the questionnaire by paper you may fill out the attached paper questionnaire and return it to your student’s school.

All day absences – Parents must call before 2:00 PM on the day of absence.
Arriving Late – Parents must call in the morning before the student arrives to put them on a late list. They must check into the Counseling Center if they are more than 10 minutes late in order to be excused. If they are less than 10 minutes late, they must go directly to class and resolve the tardy with the teacher.
Street passes – Parents must call in the morning and a street pass will be ready for the student to pick up in the Counseling Center. Street passes are not delivered to class. Please specify if the student will be returning to school.
No all-day absences will be retroactively excused after 3 days during the current term.
Written notes from parents are not accepted at Skyline!
Students must have a street pass to leave campus during the day. Leaving campus without a street pass will result in an "A," or unexcused absence.
'E' Excused absence; a parent has called the school on the day of the absence –
'R' School released absence (i.e. field trips, tournaments, etc.) – Participation points given.
'A' Unexcused absence; a parent has NOT notified the office or the absence is not justifiable. Non-justifiable absences include baby-sitting jobs, being "needed at home," "over-sleeping", and attending appointments that can be scheduled after school – No participation points awarded!
'M' Medical Excuse – Participation Points are given. Extended absences for medical reasons (5 or more days) must be excused by parent contact and a doctor's note (faxed in) and will only be accepted during the term the absence happened. No medical notes will be accepted after the term ends.
'F' Funeral – Participation points are given.
'V' Vacation, parent excused – No participation points are given.
'U' Confirmed sluff – No participation points are given.
'S' Suspension – Participation points are given.
GST2 automotive class will be offered as concurrent enrollment!
Graphics will be offered as concurrent enrollment!
