Assessments are crucial for students to see their own growth and learning as well as understand their status within grade level concepts.  Please consider carefully before exempting your student from state assessments as assessments are one of the many opportunities for students to gain skills for college and career readiness.  In the event that your student needs to be exempt from state assessments, please fill out and turn in one of the following forms to your local school before the testing window opens.

All Parental Exclusion (Opt-Out) Forms can be downloaded here for elementary and junior high/high school students. The signed and completed forms need to be submitted to the school your student attends at the earliest convenience. Copies are also available at each school location if you do not have access to a printer.
