If you would like to request special meals or meal accommodations because of medical restrictions, please print and submit our Medical Statement to Request Special Meals/Accommodations.
Students participating in the National School Lunch Program who are new to Granite School District or newly diagnosed with a medical condition or disability and who require a special diet must print and submit a Medical Statement to Request Special Meals/Accommodations to the District Food Services’ registered dietitian for approval.
Forms must be renewed by September 27 of each year in order for students to continue to receive special meals at their respective school. Please allow at least 2 weeks for processing during the beginning of the school year due to the high volume of forms typically received.
Forms must be filled out completely, otherwise they may be returned to you.
Granite School District does not have a USDA-approved fluid milk substitute.
Also, in cases where sack lunches are provided by Granite Food Services for field trips, forms MUST be on file for children who have PEANUT allergies and are going on the field trip in order for such students to receive accommodations. (Prepackaged PB & J is used most often for field trips otherwise because it does not require refrigeration.) If your child has peanut allergies, but will NOT be eating school meals other than sack lunches on field trips, please write “FOR FIELD TRIPS ONLY” on your request form.
