
Fluid Milk Substitution Requests
If your student requires a fluid milk substitution during school meals due to medical restrictions or disability, you may submit our GSD Nutrition Fluid Milk Substitute Request Form (Fluid Milk Substitute Form) below.
Please note: this form does not require a signature from a medical authority and does not cover any foods or extensive dairy restrictions.
Special Meals Requests
If your student is planning on eating school meals, and you would like to request meal accommodations because of medical restrictions or a disability, please print and submit our Medical Statement to Request Special Meals, Accommodations, Milk Substitutions Form (Special Diet Request Form) below.
Please note: this form requires a signature from a medical authority.
Because we receive many applications at the beginning of every school year, your request may take up to two weeks to process. The form must be filled out completely, otherwise it may be returned to you.
Special Diets Additional Information
Completed forms can be faxed to 385-646-4324 or sent as attachments to e-mail to Amy Pham.
- It is not necessary to provide a new special diet statement every school year if there are no new dietary changes.
- In cases where sack lunches are provided by Granite Nutrition Services for field trips, forms must be on file for children who have peanut allergies to receive accommodation. (Prepackaged PB&J is used most often for field trips because it does not require refrigeration.)
Special Diet Changes
If your student is no longer in need of a special diet accommodation, and you would like to discontinue special diet accommodations or restrictions for their meals, please print and submit the following form via email to Amy Pham.
Non-Medical Dietary Requests
For non-medical dietary requests, including but not limited to restrictions related to preference or religious or cultural practices, accommodations may be made at the discretion of Nutrition Services personnel, but is not required under federal law.
Due to the high population of students that abstain from pork products, Granite Nutrition Services regularly provides pork-free options to better serve its students.
